1. Tell us about your situation
We ask plain-English questions instead of handing you a technical PDF, and we validate your answers as you type.
2. Add family members
Include a partner, children, or a newborn. We adjust the questions and the required documents for each person.
3. Upload your documents
You get a personalised checklist based on your residency status. Upload securely, or tick the box to email them to us after checkout.
4. Review and sign
You check a full summary and sign digitally. Nothing is prepared until you confirm the details are correct.
5. We prepare your application
We review what you provided, complete the official form on your behalf, and attach your supporting documents behind it.
6. You lodge it with Services Australia
You receive your completed application by email with exact instructions on how and where to lodge it. Services Australia makes the final decision.

The official Medicare MS004 form is available free of charge through Services Australia. Our fee covers guidance, document review, application preparation, and submission assistance — not the government form itself.
